Step 1:
- Select the “Create account” button provided in one of these two ways:
- Register through an email: You will receive an email prompting you to create an account when an appointment is scheduled for you.
- Register after a visit: If you are not already registered, you will be prompted to create an account after your visit is over.
Step 2: Choose whether to verify your identity via email or text.
Step 3: Complete the multi-factor authentication by entering the code you received.
Step 4: Fill out your personal and insurance information.
Step 2: Fill out your medical history.
