Follow these steps to request a mental health visit on the patient portal
Step 1: Sign in to the patient portal using your username and password.
Step 2: Select the “Mental Health” tile.
- Note: You can see on this tile that this is “Care by Teladoc Health.” This is a mental health service offered by Teladoc Health, not your health system.
Step 3: Select yourself or a dependent to create a visit for.
Step 4: Proceed through the Teladoc Health medical intake form. You will input your desired visit time, contact information, details about the reason for your visit, pharmacy information and emergency contact.
Step 5: Complete the mental health evaluation form.
Step 6: Select the pharmacy you would like to use.
Step 7: Enter your current medication details.
Step 8: Indicate if you will require medication for the visit.
NOTE: If you select that you DO require medication, you will be connected with a psychiatrist. If you select that you DON'T require medication, you will be connected with a therapist.
Step 9: Select a provider.
Step 10: Once finished, review your details, select the optional consent and submit your visit request.
Step 11: Once your visit is confirmed, enable your microphone and video camera and join the waiting room.
