This article explains how providers and practitioner admins can mark a signed Smart Note as “Entered in Error,” and what changes occur in the system once this action is taken.
What Does This Feature Do?
Marking a Smart Note as “Entered in Error” updates its visibility across the platform to clearly indicate that the note should no longer be considered valid. This includes:
- A notification bubble on the encounter banner
- A watermark on the clinical note and any faxed copies
- Smart Note icons turn red with a tooltip that reads “Entered in error”
- The audit log records the action for practitioner admins
- The note becomes filterable in the Encounters view by status
- Can be configured in Practice Settings to allow for either the Practitioner admin or the Provider (who signed the note), to mark a Smart Note as “Entered in error”.
Who Can Use This Feature
- Practitioner Admins OR the Provider who originally signed the note (depending on what is configured in the practice-level setting)
Pre-Requisites:
- The note must be signed
- The user must be either the provider who signed the note, or a practitioner admin (depending on what is configured in the practice-level setting).
Instructions
How to Mark a Note as Entered in Error
- Open the signed note from Encounters, Scheduling, or Queue.
- Click the three dots '...' on the More button and a pop-up menu will appear,
- Select Mark as Entered in Error.
- A confirmation popup will appear. Click Yes to proceed. ⚠️ This action cannot be undone.
- The note is watermarked with “Entered in Error.”
Filter on Encounters
- The right hand side of encounters filter now includes 'Entered in Error' as an option for Consult Note status'
Confirmation for Entered in Error
Once a note is marked as entered-in-error, users will see the following changes in the user interface.
- The note is watermarked with “Entered in Error.”
- The Smart Note icon turns red.
- Hovering over the icon displays the tooltip: “Entered in error.”
- A red notification bubble appears on the encounter banner.
- Clicking the bubble reveals additional details.
- Faxed versions of the note will include the watermark.
